Team Nurse Coordinator
Description: The Home Team Nurse Coordinator is responsible to the Director of Clinical Services for supervision and coordination of the delivery of patient/family nursing care and care-plan coordination functions with other members of a dynamic interdisciplinary team.
Qualifications: Must have at least a Bachelor’s Degree in Nursing with previous management experience and at least two years in hospice and palliative care.

Registered Professional Nurses
Description: Home Care (Full Time, Part Time, Weekend)
Qualifications: B.S Degree in Nursing preferred; Hospice Nursing preferred
